Join Tara’s Pack in Five Easy Steps

Tara’s Place takes the utmost care in maintaining an extremely clean safe haven for your special pets. We must all work together in order to provide top industry standards every day.

You must follow these 4 easy steps prior to your pet’s first day with us:

1. Print and fill out this form: YOU'RE INVITED!

2. Email it to info@tarasplace.net or fax 706-757-3890 or bring it in.

3. Send us your pet's VACCINATION RECORDS.

4. Call us to book your EVALUATION VISIT.

5. Sign up your pet to attend DAYCARE or SLEEPOVERS at Tara's Place.

YOUR PET MUST HAVE TAG(S) OR THERE WILL BE A $3.00 TAG FEE.

All Dogs 7 months and older must be spayed or neutered.

Tara’s Place only accepts healthy, fully-vaccinated dogs and cats. Animals with communicable diseases of any kind are NEVER accepted.

YOUR PET MUST BE ON FLEA AND TICK PREVENTATIVE

	Tara's Place welcomes unaltered puppies up to 7 months of age who have received their complete round of puppy shots, plus Bordetella. No exceptions. Puppies 7 months and older must be spayed or neutered as well.
	Feline guests over 4 months must be spayed or neutered to Sleepover.
	You must submit copies of your pet’s veterinary health records to Tara’s Place. Complete records may be faxed to Tara’s Place at 706-262-2811 or our staff can speak directly with your veterinarian prior to your EVALUATION VISIT.

Vet records must prove that the following vaccinations have been administered within the last 12 months:

Dogs:
Rabies
DHLPP (Distemper, Hepatitis, Leptospirosis, Parvo & Para Influenza)
Bordatella (aka Canine Cough)

YOUR PET MUST BE ON FLEA AND TICK PREVENTATIVE

Cats:
Rabies
FCRVP (Feline Leukemia)

Your Dog’s Evaluation Visit

PLEASE NOTE: Vet records MUST be submitted PRIOR to your dog’s Evaluation Visit

All first timers to Tara’s Place must attend a $10 Evaluation Visit, which REQUIRES a minimum of 2 hours of DAYCARE.

*PLEASE NOTE: Your pet MUST have an ID tag. If not, Tara’s Place will supply one and charge you a $3.00 fee.  This is for a temporary tag to be worn for the visit only.

We want to make sure your dog will love it here, so it’s in your best interest to allow us to assess your dog’s temperament and behavior in a pack setting.

	Overly dominant and/or aggressive dogs, toy-possessive and/or food-possessive dogs are NEVER accepted to ensure the safety of every pet and our staff.
	Once your dog has passed our Evaluation Visit, you are free to make reservations for DAYCARE and/or SLEEPOVERS.

If your dog does not pass the Evaluation Visit, you will be notified immediately and they will not be able to stay at Tara’s Place.  We can board your pet without the use of daycare if that is desired.

C’mon Back and Re-join the Pack

After your dog’s first regular DAYCARE or SLEEPOVER stay, all future bookings may be done over the phone with no Evaluation required.

Vaccination records must always be confirmed as current and any updates faxed to us prior to your visit.

A credit card is required to reserve all bookings.

We require NO LESS than 72 hours notice for cancellations. Otherwise you will be charged for 1 nights stay.
