Your Pet's Vaccination & Health Information*
Bring in a Copy of your pet's vaccination records with you to Tara's Place. Please Note: Tara's Place only accepts healthy, fully-vaccinated dogs and cats. Animals with communicable diseases of any kind are NEVER accepted. Please keep your pet at home if they are displaying any signs of illness or ill health including diarrhea, loose stool, non-allergy related eye discharge, vomiting, etc. OR YOU WILL BE HELD FINANCIALLY RESPONSIBLE FOR THE VETERINARY TREATMENT OF ALL PET GUESTS WHO BECOME SICK DUE TO YOUR PET'S VISIT/SLEEPOVER.
Tara's Place does accept healthy dogs who require regular medication. Please note that clients will be charged veterinarian fees for any injectable medications required.

At Tara's Place, we really care about our Guests and want them to feel at home. We encourage you to call us at 706-757-3890 any time during business hours to check in on your pet! *PLEASE NOTE: Your pet MUST have an ID tag. If not, Tara's Place will supply one and charge you a $3.00 fee. This is for a temporary tag to be worn for the visit only.

Our Promise to You: Tara's Place will provide a safe, clean & loving second home for pets who need daycare and/or boarding and thrive on the socialization of indoor/outdoor group play. We will only employ individuals with a deep passion and understanding for animals & promise to always treat "guests" & their owners with the utmost respect and kindness. Thank-you for noting the following Tara's Place policies. Please initial your understanding: We strongly urge you to bring your pets usual food for their stay. 1. All pets must arrive on a leash or in a crate, under your control.	Initial Here
2. Check-out time is 11 a.m. Your pet must be picked up by then to avoid a fee, equivalent to one day of daycare.	Initial Here
3. Your pet must have an ID tag (or a rabies tag) or there will be a $3.00 temporary tag fee.	Initial Here
4. Your pet must be on flea and tick preventative. If fleas or ticks are found on your pet – they will be bathed and you will be billed for a medicated flea bath.	Initial Here
5. All cancellations must be made by phone no less than 72 hours from your first reserved day. Otherwise a fee will apply, equivalent to a 1-night stay.	Initial Here
6. Our hours are: Mon.-Fri. Pick-ups/Drop-offs - 7:15 a.m.-12 noon & 2 p.m.-6:45 p.m. Weekday Daycare – 7:30 a.m.-12 noon & 2 p.m.-5 p.m. Sat. Pick-ups/Drop-offs – 10 a.m.-4 p.m. (by appointment only) Tara's Place is closed Mon.-Fri. 12 p.m.-2 p.m.	Initial Here
7. Tara's Place provides raised comfort cots in each of our standard bunks. If your pet chews their cot, you will be charged a replacement fee of $45	Initial Here

The important legal stuff
Please read the information below and sign here to give Tara's Place permission to care for your loved one: This is a binding contract between Tara's Place, LLC and the Pet Owner: Pet Owner Date
I certify that I am the Owner or Guardian of the aforementioned pet(s) and that I am authorized to bring them in for daycare/boarding and sign this form. I understand that although Tara's Place, LLC will exercise reasonable care to all pets staying at Tara's Place, LLC, pet boarding/daycare is not without some risk. While under the care of Tara's Place, LLC, I agree to be solely responsible for any and all acts of this dog/cat(s) behavior. I assume any expense and liability for injuries to dog/cat(s) and/or persons while at Tara's Place, LLC. In addition, I agree to pay all medical and/or veterinary expenses incurred as a result of any injury inflicted by this pet to another person or dog/cat(s) while under the care of Tara's Place, LLC I certify that this pet is completely healthy and free from any communicable illness. I understand that if my pet is found to have such an illness, I will be called to remove the pet from the premises immediately and will be held financially responsible for all pets who have become ill due to this pet's stay at Tara's Place, LLC Guardian gives Tara's Place, LLC sole discretion to act on Guardian's behalf in electing whether or not to seek veterinarian care for Guardian's dog/cat(s) at Guardian's expense. Guardian hereby waives and releases Tara's Place, its employees, owners and agents from any and all claims. I authorize Tara's Place, LLC to contact my veterinarian in order to confirm health, temperament and/or vaccinations. I give consent to Tara's Place LLC to act on my behalf in obtaining veterinary care at my expense, should Tara's Place LLC deem it necessary. I authorize Tara's Place, LLC to charge my credit card for any outstanding invoices. I hereby waive and release Tara's Place, LLC, its employees, owners and agents from any and all liabilities or claims while inside the building, or on the grounds and the surrounding area, and resulting from participation at Tara's Place, including specifically, but not limited to, any injury, damages or death of this pet, resulting from their stay at Tara's Place, LLC. I understand that under no circumstances will Tara's Place be liable for consequential damages or damages beyond the replacement value of my pet. Thank-you for choosing Tara's Place.
